How to Apply for Funding

Grant Application Process

Komen Tulsa issues a Request for Application (RFA) to organizations that provide breast cancer programs in Tulsa County on an annual basis. RFAs are reviewed by an independent, anonymous third-party who will be looking for organizations that can demonstrate innovative projects in the areas of breast health and/or breast cancer education, outreach, screening or treatment support. This includes projects targeting services not otherwise available to the medically underserved populations in Tulsa County.

For 2011 the Komen Tulsa grant cycle will run from April 1, 2011 through March 31, 2012.

Thank you for your interest in the Komen Affiliate grants program. Please complete the form below to receive information about the Komen Affiliate grant program, including announcements, Request for Application (RFA) information and networking event information. To be eligible to apply for a Komen Community-Based Grant, your organization must be:

  • A federally tax-exempt entity — nonprofit organization, government agency, educational institution or Indian tribe.
  • Located in the specified affiliate service area.

Grant Cover Page - Required for all applications. Includes information about the applicant.
Budget Template - Worksheet for funding request.
Community Profile- All grants will be reviewed against the community’s needs identified within the Community Profile.
Educational Materials and Messages - Please review and ensure that your organization’s message is consistent with Komen’s educational materials and messages.
